Two minor issues in the INaK model implementation.

There are two minor issues in the INaK model implementation. Neither has a significant effect on the model predictions under baseline conditions.

1. The equation for x1 should be x1 = a4 * a1 * a2 + b1 * b4 * b3 + a2 * b4 * b3 + b3 * a1 * a2, where the correction is the "b1" in the second term (the paper and code erroneously use a "b2" here).
2. The [H+] is set to 10^-7 [mM], i.e. 10^-10 [M] for a pH of 10. Presumably this should be 10^-4 [mM] for a pH of 7.
